Filing for orthodontia reimbursement

Standard Method: For new orthodontic contracts, Flex-Plan Services will reimburse participants as payments are made. If the participant prepays the entire amount to get a discount they will be reimbursed the full amount. Also, participants who make a down payment or make monthly payments will be reimbursed the amount they paid within the plan year. In no case will FPS reimburse Late Fees or Finance Charges.

New Participant Method: Participants who have already paid the full amount of their orthodontic contract prior to beginning with FPS may wish to be reimbursed for their expense. In order to be reimbursed, the participant will need to provide a statement from their orthodontist showing how much of the payment would have been applicable to their current plan year with FPS, as well as proof of payment.
