Dependent Care FSA

Accessing Your FSA

Accessing your Dependent Care FSA is easy:

  1. Obtain a receipt from your dependent care provider. The receipt must include the dates of service, type of care, cost, and the dependent's name.
  2. Fill out a claim form.
  3. Fax, email or mail your claim and receipt to Flex-Plan Services.
  4. Reimbursements will be issued according to your employer’s schedule.

Your Dependent Care FSA functions like a bank account--you will only be reimbursed up to the current balance in the benefit.

If your dependent care provider does not offer formal receipts, you may use this form to document services: Day Care Service Form
